  1  How to receive a copy of Birth certificate, Marriage certificate or a Death certificate

• Where the date of registration or the number of the entry is given the fee for one copy of the certificate is Rs. 100.00
•  Where the date of registration or the number  of the entry is not given  involved fee for one copy of the certificate is Rs. 200.00
• Required fee should pay to the Registrar Division
• Duly filled application should submit to the Registrar Division

2   Registration of Marriages

1. Exchange the Notice of Marriage. (The" Notice of Marriage" can be taken from the registrar division )
2. Submit the Notice of Marriage berofe register the marriage (approximately 14 days).
3. Present Grama Niladhari Certificate to confirm the residence
4. Present the documents to proof the age.
   
   • National Identity card
   • Birth certificate
    
5. Fees for Registration of Marriages
   • submit the Notice of Marriage  -   Rs: 100
   • Issue the Marriage certificate  -   Rs: 100
   • To registar the Marriage          -   Rs: 750

        * * Emergency marriage registration

3   Correction of errors in birth certificates

1. Ammending the name in a birth certificate.
   
2. Ammending the details of mother.
3. Ammending the details of father.

1. Declaration completed under article 27
2. First copy of the relevent birth
3. An affidavit
4. Grama Niladhari Certificate
5. Request letter
6. Written wittness for expected name which to be used.
   • An Extract of electoral registar
   • Bank account book
   • paper notice
   • Marriage certificate of parents
     
   • Birth certificates of children
   • Copy of deeds
   • Other acceptable witnesses

II   Ammending the details of mother

1. Declaration completed under article 52(1)
2. First copy of the relevent birth certificate
3. An affidavit
4. Grama Niladhari Certificate
5. Request letter
6. Marriage certificate of parents
7. A name list of brothers and sisters
8. Birth certificates of brothers and sisters
9. National Identity card
10. Others

III   Ammending the details of father

1. Declaration completed under article 27(අ)
2. First copy of the relevent birth certificate
3. An affidavit
4. Grama Niladhari Certificate
5. Request letter
6. Marriage certificate of parents
7. Father's birth certificate
8. A name list of brothers and sisters
9. Birth certificates of brothers and sisters
